Care Management Organization (CMO)

Overview

BFDI CMO is contracted to provide supervision and monitoring of treatment services to youth and families that are placed with the Wayne County Department of Children and Family Services (WCDCAFS).

CMO Program Goal

To ensure youth and families receive services they need to be successfully released from the juvenile justice system and achieve their personal goals, while ensuring community safety.

CMO Program Services

  • Reintegration of youth in the community
  • Improve independent functioning
  • Refer and Monitor treatment services
  • Educational Monitoring
  • After school support services
  • Advocacy
  • Resources

    Eligibility Criteria
    To be eligible for this program a youth must:
  • Be Court ordered
  • Be 7- 21 years old
  • Live in Wayne County
  • Generally reside in zip codes: 48205, 48211, 48213, 48214, 48215, 48224, 48225, 48230, 48236
